FSL Grade Release Requests
Fraternity and Sorority Members
Eligibility for membership and/or eligibility to hold leadership positions within a recognized fraternity or sorority may be based upon your semester and cumulative grade point averages at the 91PORN.
FERPA, The Family Educational Rights And Privacy Act of 1974, requires your permission to release grade information to third parties.
The following electronic release forms allow you to give permission to the 91PORN to release your grade information.ÌýThere are two forms below. Please use the form that aligns with yourÌýaffiliation.Ìý
If you are a member of the College Panhellenic Council, National Pan-Hellenic Council, Multicultural Greek Council or CU Interfraternity Council, please use the following form.
If you are a member of IFC on the Hill, please use the following form.

If you are no longer a member of a fraternity or sorority, you may revoke your grade release. Do NOT submit the following form if you are still a member of a sorority and wish to remain a member.
